Italian Conte is only the fourth manager to win the Premier League title at the first attempt, emulating his Stamford Bridge predecessors Jose Mourinho and Carlo Ancelotti as well as Manuel Pellegrini who achieved the feat with Manchester City.

Antonio Conte was under pressure at the start of the season.

Costa was the life and soul of the post-match party, another testament to Conte, who has got the striker happy and playing close to his best again this season.

Moses won his first-ever Premier League title following Chelsea's 1-0 win against West Bromwich Albion at the Hawthorns on Friday.

10th last term as Leicester succeeded them as champions - take an unassailable 10-point lead over Spurs with two games to spare, clinching the Blues' fifth Premier League title in 13 seasons. "We've worked hard and there's no better feeling in football".

"You have to cherish these moments, you work so hard all season to be where we are", said Chelsea defender and stand-in captain Gary Cahill.

"We did feel a bit nervous because of the pressure to be champions".

Conte then reportedly told his Chelsea players in the dressing room: 'You must show on the pitch you are a good team, you are not a good team just because you are at Chelsea'.

It was not vintage Chelsea on Friday but they got the job done on a tense night in the Midlands.

Eden Hazard had another effort blocked on the line but as Chelsea strained for the victor they began to look a little vulnerable at the back as West Brom gained in confidence.

Despite their dominance of possession, Chelsea still had to be wary of their hosts' countering capabilities and, left one-on-one with David Luiz, Rondon slipped clear and raced towards goal, only to be denied by a fine block from Azpilicueta.

There's much more root of optimism, that this club hasn't had since I've been here.

But with the hard-working Conte at the helm, expect Chelsea to overcome the challenges.

"I am happy. Very happy".

"The first title felt wonderful and to win it again is unbelievable".

"When I chose to come back here I dreamed to win the Premier League".

"This is a dream that has come true".

Their remaining games are at home to Watford on Monday before entertaining Sunderland a few days later where they'll receive the Premier League trophy.

Conte's side will now aim to add another title in their account when they face Arsenal in the FA Cup final on May 27.

For a club that can be toxic and leaky, there have been few flashpoints this season in the first year under Conte.
