A source told the Journal that he had no cellmate and wasn't being checked every 30 minutes.

On the morning of Epstein's death there was shouting and shrieking from his jail cell, a source familiar with the situation told CBS News.

Last month, Epstein was found unconscious on the floor of his jail cell with marks on his neck, and officials were investigating that incident as a possible suicide or assault.

"Let me assure you that this case will continue on against anyone who was complicit with Epstein", Barr said while addressing the Fraternal Order of Police in New Orleans. Additionally, the Bureau of Prisons placed the two MCC staffers assigned to monitor Epstein on administrative leave as the FBI and DOJ inspector general begin intensive investigations into how such a high-profile inmate had the apparent means and opportunity to kill himself.

The warden who was supervising the center has been reassigned to the Bureau of Prisons' Northeast Regional Office, according to the Justice Department. "FCI Otisville Warden James Petrucci has been named Acting Warden of the MCC New York".

Taken together, the deaths underscore "serious issues surrounding a lack of leadership" within the BOP, said Cameron Lindsay, a former warden who ran three federal lockups, including the Metropolitan Detention Centre in Brooklyn. Teachers, nurses, counselors, secretaries, and other support staff are regularly obligated to fill in for guards, many of whom work 70- to 80-hour weeks, according to Eric Young, president of the American Federation of Government Employees Council of Prison Locals union.

Epstein was being held without bail, awaiting trial on federal sex trafficking and conspiracy charges that could have brought 45 years in prison.

Federal prosecutors in NY are pursuing a parallel investigation into whether any if his associates will face charges for assisting him in what authorities say was his rampant sexual abuse of girls as young as 14.

According to police reports, Federal Bureau of Investigation records and court documents, Epstein had a team of recruiters and other assistants who knew of his penchant for girls and lined up victims for him.

According to the report, federal prisons routinely face shortages of fully-trained guards and have been known staff under-qualified workers.

An inmate in the same unit where Jeffrey Epstein hanged himself heard nothing the morning of his death - but "knows a heck of a lot about what went on", his lawyer said in a new report.

Maxwell is said to be Epstein's ex-girlfriend turned business associate.

"We are now learning of serious irregularities at this facility that are deeply concerning and that demand a thorough investigation", Barr said.

The Metropolitan Correctional Center where financier Jeffrey Epstein was being held, on August 10, 2019.

The move would have had to have been authorized by the facility's suicide prevention program coordinator, who is usually the chief psychologist.

The New York City medical examiner said that an autopsy had been completed on Epstein on Sunday but that a determination on the cause of death was still pending. "This is disgraceful, not only for the justice system, but for the hundreds of victims".

US President Donald Trump has commented on the case, saying he would like to see an official probe into Epstein's death.
